Complaint Form
If you wish to file a formal complaint about a business, please use our form. Before using this form, complain directly to the company. If unsuccessful, then use this form.
This form is only for use in Ulster County, NY; you must be a resident or have a complaint about a company doing business in Ulster County.
You will be asked to attach photocopies of any papers involved in the transaction. (Including contracts, receipts, statements, the front and back of canceled checks, correspondence, warranties, et cetera). DO NOT SEND ORIGINAL DOCUMENTS. The bureau will not be responsible for originals.
