Child/Family Case Management SPOA

The Child & Family SPOA offers personalized care coordination to youth with Serious Emotional Disturbances (SED) and their families. 

Single Point of Access (SPOA)is a New York State Office of Mental Health initiative that facilitates timely access and linkage to care management and other supportive services.

SPOA coordinates among care providers and streamlines referrals to services. This “no wrong door” approach makes it easier and less stressful to access a full range of services.  Referrals are tailored and responsive to individual needs and preferences.

Step 1.Confirm Your Eligibility

SPOA services are intended for youth with Serious Emotional Disturbances (SED). If you’re unsure whether you qualify, talk to your healthcare provider.

Step 2.Gather your documentation

We ask that your SPOA application is accompanied by supporting documentation from your mental health provider. This should include:

  • a copy of the most recent psychiatric evaluation
  • psychosocial assessment
  • written narrative with service needs
  • any other supporting documentation that may be helpful in connecting you to the most appropriate service(s)

Step 3.Complete the application

In order to be considered for SPOA services, please complete the application fully and submit on our website or via fax at (845) 340-4192 or e-mail at dmh@ulstercountyny.gov

Be sure to sign all applicable consents which will allow us to refer you to requested services and obtain other relevant information from past providers.

Step 4.Be ready for our call!

Our staff will reach out to you with any questions or requests for additional information, and to speak to you about available services and resources. Please be sure the best ways to reach you are on the application. 

Services

The Child & Family Case Management SPOA coordinates referrals to the following services:

  • Family Peer Support, provided by certified Family Peer Advocates who have lived experience raising a child with behavioral health needs and navigating the system of care. Advocates meet with families weekly.
  • Youth Respite
  • High Fidelity Wraparound
  • Coordinated Children's Services
  • Multisystemic Therapy (MST)
  • NEXIS
  • Mental Health in Schools
  • Health Home Care Management,  available to youth with SED or 2 qualifying health conditions. Frequency of contacts are based on level of need.
  • Non-Medicaid Care Management, a service for youth who are not eligible for Health Home. It is for youth diagnosed with Serious Emotional Disturbance (SED). Care coordinators meet with youth and families 2 times per month to work toward recovery-oriented goals.
  • The Getting Ahead & Transitions Programs provide care management services for transitional-aged youth who are not Health Home eligible. Care coordinators meet with youth two times per month to work on goals related to becoming more independent.
  • Home & Community Based Services are also available to youth with Medicaid and provide additional specialized services.
