Long-term Care

Long-term care services refers to the wide range of in-home, community-based and institutional services and programs that are designed to help the elderly and individuals with disabilities of all ages.

Throughout the state, the New York State Office for Aging and the Department of Health work with local Offices for the Aging, Social Services and Health Departments to empower individuals to make informed choices and to streamline access to long-term care services.

Personal Care / Home Care

OFA partners with local providers to offer Personal Care Aide services to eligible clients. These services are provided in the homes of seniors with temporary or chronic needs who cannot perform day-to-day tasks. Each client is additionally assigned an OFA Case Manager.

Case Management for Seniors

These services include the Expanded In-Home Services for the Elderly Program.
