Long-term Care

Long-term care services refers to the wide range of in-home, community-based and institutional services and programs that are designed to help the elderly and individuals with disabilities of all ages.

Throughout the state, the New York State Office for Aging and the Department of Health work with local Offices for the Aging, Social Services and Health Departments to empower individuals to make informed choices and to streamline access to long-term care services.
