The Children and Youth with Special Health Care Needs (CYSHCN) Program connects families with the services, resources, and support they need to help their children stay healthy and reach their full potential.
If your child has a chronic physical, developmental, behavioral, or emotional condition, we can help you find services, navigate systems, and connect with community resources.
Who Can Receive Services?
Services are available for children and youth from birth through age 21 who have, or are at increased risk for, a chronic physical, developmental, behavioral, or emotional condition and may require services beyond those typically needed by children.
Services We Provide
We can help you:
- Connect with health care providers and specialists
- Find community programs and support services
- Access educational, developmental, and disability resources
- Connect with family support and advocacy organizations
- Receive information and referrals based on your family's needs
- Support youth ages 14-21 with information to transition from pediatric to adult health care services
- Connect youth with transition information and services to prepare for life after graduation
