Public Pool, Beach, Campground and Temporary Residence Permits

Permits are required to operate:

  • Swimming pools (except those owned and/or maintained by an individual for the use of his family and friends)
  • Bathing beaches
  • Spray grounds
  • Temporary residences
  • Campgrounds 

These facilities are regulated by the Ulster County Department of Health under NYS Sanitary Code. You must have a permit to operate these facilities. Operating without a valid permit may subject you to an administrative enforcement action and may result in closure.

Applications and the appropriate fee(s) must be submitted at least 30 days before the first day of operation or expiration of the existing permit. 

Instructions

Step 1.Review Cover Letter

Before you begin, carefully read the cover letter of the application for your type of facility.

Step 2.Gather Insurance Documents

New York State Law requires every facility to have workers’ compensation and disability insurance or to have aCE-200 Exemption form from the New York State Workers’ Compensation Board. Learn more about required insurance forms and gather your documents.

Step 3.Complete Application

Complete sections A, B, F, G, & H of the Application for a Permit to Operate (DOH-3915).

Step 4.Safety Plan Certified Statement

If your facility includes a pool/beach or spray pad, you must submit a S(PDF, 77KB)afety Plan Certified Statement(PDF, 77KB).

Step 5.Water Sample

For seasonal facilities with their own water systems, a completed Water Start Up Certification(PDF, 31KB) and satisfactory bacteriological and nitrate sample results are the be submitted to this department before the permit can be issued.

Step 6.Prepare Payment

Refer to the cover letter to determine your application fee. Include water sample fee and late fee, if applicable. 

You may pay by cash, check, or money order. If paying by check or money order, make payable to: Ulster County Commissioner of Finance.

ALL APPLICATION/PERMIT FEES ARE NON-REFUNDABLE

Step 7.Submit Application

You may send your application by mail, or deliver it in person during our business hours to:

Make sure to include:

  • Fully completed application, including signature on page 4
  • Insurance documents (WC/DB or CE200)
  • Fee
  • Safety Plan Certified Statement and/or water sample, if required.
  • Any other information or documentation specified in the cover letter.

The application cannot be processed if ANY of the required information is missing or incomplete. Incomplete applications will be returned and subject to a $75.00 late fee, if applicable.

Step 8.Pre-Operation Inspection

Once your application is received, the Department of Health will inspect your facility.

Step 9.Permit Issued

If the preoperational inspection is satisfactory, the Department of Health will issue your permit.

New York State Law requires every facility to have workers’ compensation and disability insurance or to have a CE-200 Exemption form from the New York State Workers’ Compensation Board.

If your facility requires workers' compensation and disability insurance:

  • For Worker's Compensation, you must provide one of the following:
    • Form C-105.2 — Certificate of Workers’ Compensation Insurance (issued by your insurance carrier)
    • Form U-26.3 — Certificate of Workers’ Compensation Self-Insurance (issued by the State Insurance Fund)
    • Form SI-12 — Certificate of Workers’ Compensation Self-Insurance
    • Form GWI-105.2 — Certificate of Participation in Workers' Compensation Group Self-Insurance
  • For Disability Benefits, you must provide one of the following:
    • DB-120.1Certificate of Disability Benefits (issued by your insurance carrier)
    • DB-155 —Certificate of Disability Benefits Self-Insurance

If your facility is exempt:

If your facility has no employees, you may be eligible for an exemption.  To qualify, you must obtain a signed CE-200 Exemption from the New York State Workers’ Compensation Board.
