Children and Youth with Special Health Care Needs (CYSHCN) is a comprehensive resource and referral program for families with children from birth to 21 years who have a qualifying chronic physical, developmental, behavioral, or emotional conditions.
We link families to community-based agencies and service providers that can help address your unmet needs. We also connect families to support networks that understand the unique challenges of caring for children with special needs.
For youth ages 14-21, the program provides information about the transition into adult health care.
