Adult Case Management SPOA

SPOA (Single Point of Access) is a New York State Office of Mental Health (OMH) initiative that facilitates timely access and linkage to care management and other supportive services for adults, youth and families. SPOA services are designed to promote recovery and are personally tailored and responsive to individual needs and preferences.

Ulster County has separate Case Management SPOA committees for adults and children.

Step 1.Confirm Your Eligibility

SPOA services are intended for adults with Serious Mental Illness (SMI). If you’re unsure whether you qualify, talk to your healthcare provider.

Step 2.Gather your documentation

We ask that your SPOA application is accompanied by supporting documentation from your mental health provider. This should include:

  • a copy of the most recent psychiatric evaluation
  • psychosocial assessment
  • written narrative with service needs
  • any other supporting documentation that may be helpful in connecting you to the most appropriate service(s)

Step 3.Complete the application

In order to be considered for SPOA services, please complete the application fully and submit on our website or via fax at (845) 340-4192 or e-mail at dmh@ulstercountyny.gov

Be sure to sign all applicable consents which will allow us to refer you to requested services and obtain other relevant information from past providers.

Our staff will reach out to you with any questions or requests for additional information, and to speak to you about available services and resources. Please be sure the best ways to reach you are on the application. 

Adult SPOA

Adult Case Management SPOA refers to:

  • Assertive Community Treatment (ACT)
  • All levels of case management, from Health Home Care Management to Intensive Case Management
  • Community Psychiatric Support & Treatment
  • Other specialized services, including Psychosocial Rehabilitation and Peer Support. 

SPOA also makes direct linkages to care coordination organizations to assist people with intellectual and developmental disabilities.

SPOA also offers specialized programs, including the Department’s Jail Re-Entry Case Management program, Mobile Mental Health Team and OnTrack NY, a program that serves young people who are experiencing their first psychotic episode.
